Criminal Record Check:

As per BCLA's policy, all coaches who coach athletes 18 years or age or younger must submit to a criminal record check.  Failure to provide a criminal record check could result in the individual being suspended from coaching.  

Please print out these 3 forms, fill them out and take them in person to the Public Safety building located at:
